Consent to Telehealth

THRYV Collective (“THRYV”) offers secure online platforms that allow you to connect with our licensed healthcare providers for virtual consultations. These visits may include the option to receive prescriptions from a mail-order pharmacy when clinically appropriate based on your individual needs or diagnosis. (The “Services”).

These Services constitute telehealth. The delivery of healthcare through secure electronic communication between a provider and patient who are not physically together. Telehealth can be used for diagnosis, treatment, follow-up, and patient education. “Telehealth” means using real-time or stored telecommunications technology to provide healthcare services, including (but not limited to) assessment, diagnosis, consultation, treatment, and patient monitoring; transmission of medical data; patient and provider education; public health services; and administrative functions. This definition does not include audio-only calls, standard email messages, or fax transmissions.

Telehealth services may include:

  • Secure electronic transmission of medical records, images, health data, or other information between you and your provider.
  • Communication through video, audio, secure messaging, or email.
  • Use of data from remote monitoring equipment or connected medical devices.
  • Privacy and security measures built into the systems and networks to protect your health information.

Potential Benefits of Telehealth

  • Increased convenience and reduced travel or wait time for medical care.
  • The ability to schedule care at times that work best for you.
  • A telehealth provider may evaluate your condition without requiring an in-person exam, if clinically appropriate.

Potential Limitations of Telehealth

  • Information sent electronically may not be sufficient for clinical decision-making in some cases, requiring an in-person exam or alternative care.
  • Certain tests or in-person assessments cannot be performed remotely, which may limit diagnosis or treatment options.
  • Technical issues could cause delays in care or loss of information.
  • As with any electronic system, there is some risk of data security breaches despite safeguards.
  • Legal or regulatory restrictions in your state may limit treatment options, including specific prescriptions.

Your Acknowledgment and Consent

By checking the “Agree” box, you confirm the following:

  • You understand that emergency care cannot be provided through these Services. In the event of an emergency, you should call 9-1-1 or seek immediate medical attention.
  • You consent to receiving medical evaluation and treatment through telehealth from a licensed provider.
  • You understand telehealth technology may evolve and could include tools not specifically described in this document.
  • You acknowledge that technical failures could interrupt care or result in the loss of information, and you release WePellet from liability for such issues.
  • You understand that telehealth may offer benefits but that results cannot be guaranteed. Your condition may improve, stay the same, or worsen.
  • You agree to provide complete and truthful information about your health history and to update your provider on any changes.
  • You understand that withholding or misrepresenting health information could result in harm, including serious injury or death.
  • You have the right to access your telehealth records in accordance with applicable law.
  • You acknowledge that your provider may determine telehealth is not suitable for your condition and may recommend in-person care.
  • You understand these Services are intended to coordinate care with, not replace, your existing healthcare providers.
  • You consent to the collection, use, sharing, and protection of your information as described in the THRYV Collective Privacy Policy.
  • You understand that your telehealth records may be shared with other providers only at your request and with your consent.

You may withdraw your consent at any time by contacting THRYV Collective at info@wepellet.com. Unless withdrawn, this consent automatically renews with each telehealth visit.

By agreeing, you authorize THRYV Collective and its providers to collect, use, and share your information, including personal health data related to your telehealth exam, as described in the Privacy Policy and as otherwise allowed by law for treatment, payment, and healthcare operations.
